Looks to be mechanical failure. I bet they knew the lift was in bad shape, but continued to use it anyway.There must have been stress cracks or signs of loosness prior to this that were ignored. I just can't see all the welds snaping with no previous signs of weakness. I just hope that car was not already sold.
